Cygnet Supported Living works with adults with mental health problems, learning disabilities, autism (including Asperger's Syndrome) and adults with complex needs.
We offer a range of services including community supported living and outreach services. We place a particular emphasis on providing services that promotes individual's independence, lifestyle choices and social integration in the community setting of their preference.
Recovery and community integration is at the heart of our services, we empower each individual to be actively involved in all parts of their care, and to provide real opportunities for them to develop satisfying and meaningful roles within their communities.
For some individuals especially those who currently reside in hospital placements particularly those in secure care, the pathway to achieving support within a home of their own choice is often problematic and fraught with difficulties over which they have no input or control.
Cygnet Supported Living have developed a specific Community Focused Recovery Pathway, for those individuals who find themselves in hospital and group home settings where accessing appropriate levels of community support, specific to their needs has not been available or straightforward to obtain.
